DESMOND Type 2 Diabetes Education - Nottinghamshire County GP Practices

Nottinghamshire Healthcare NHS Foundation Trust has offered DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed) for structured Type 2 Diabetes patient education since 2015.

DESMOND is a structured education programme for adults diagnosed with type 2 diabetes and their family members or carers. As part of diabetes care, structured education should be offered at diagnosis and reinforced annually. Therefore, the groups will be a mix of people with a new diagnosis and others will have been diagnosed for many years. The DESMOND programme is delivered to anyone with a type 2 diabetes diagnosis, regardless of how it is being treated.

The course is delivered by two friendly educators and has an interactive approach to learning. A DESMOND course lasts 6 hours, either 2 half days on consecutive weeks, or one full day.

Referral criteria:

Over the age of 18 years old, Diagnosis of type 2 diabetes, Nottinghamshire County GP

Localities:

The team provide a face-to-face service across Nottinghamshire County; Mansfield & Ashfield, Newark & Sherwood, Rushcliffe, Broxtowe & Gedling.

Virtual courses are also regularly delivered.

Times available:

Office - Monday to Friday - 08:30 to 16:30.

DESMOND courses delivered Monday to Saturday (morning, afternoon or full day)

Times and availability
