What is the Notts NHS App - Personal Health and Care Record?

Your Notts NHS App - Personal Health and Care Record is an online record owned by you. It is a simple (and secure) way that allows you to add, manage and organise your health and wellbeing information on your smartphone or tablet device.

It gives you the ability to make appropriate parts of it available to those who need it such as loved ones, healthcare professionals and other health organisations.

How do you create your Personal Health and Care Record?

Your Personal Health and Care Record is created by linking two systems to work together:

  • the NHS App
  • Patients Know Best (PKB)

To link the two systems and access all the features of both systems through the NHS app all you need to do is register once for the NHS App and Patients Know Best.

How to register for the NHS App?

The NHS App is free to download from the App Store or Google Play and can be used by anyone who is registered with a GP surgery in Nottingham or Nottinghamshire. If you don't own a smartphone or tablet you can still access all the features  of the NHS App online through your computer or laptop. 

If you're aged 13 to 15, you'll need to visit your GP surgery first and request access to the app before you can use it.

Once downloaded, you can set up your NHS login if you have not already got one as shown in the video below. Stage 1 in the video would be you choosing to open the NHS App.

Or use the details your GP surgery gave you when you registered for their GP online services.

How do you register for Patients Know Best (PKB)?

When your GP Practice goes live with this service, you will receive an email or a text inviting you to register. So, it's really important you make sure your GP has your up to date email address and mobile number.

Follow the link in your email to register for PKB.

When you have registered to both, you will need to log in via the NHS App on your smartphone or tablet.

Once you have done this you will automatically be able to access the Patients Know Best service using the NHS App.

What does Patients Know Best do?

Patients Know Best (PKB) provides you with all your health and care information in one place and puts you in control of who can see it. It means you can see your information wherever you are, whenever you need to.

Patients Know Best (PKB) allows you to see your information wherever you want to, whenever you need it, in one place. You will have access to:

  • Hospital and healthcare appointment letters
  • The ability to virtually access care plans
  • library of local information resources to help manage your condition(s)
  • Consultations virtually via questionnaires, video, voice or messaging
  • The ability to self-monitor and track symptoms and test results
  • Attach medical and fitness devices to pull information into your app automatically
  • Seamlessly share information with family, carers or any health and care professionals anywhere if you choose to
  • Ordering repeat prescriptions
  • Book and managing appointments
  • Secure messaging to your healthcare provider's team
  • Viewing key information about your health and care

Your PKB account is yours for life. All your health and care information will continue to be fed in from all participating healthcare providers so you have a historical record of your health information all in one place.
