Community Neurology Service

The Community Neurology Service provides support people to manage a long term neurological condition and to live as independently as possible. We aim to increase the number of people with a long-term neurological condition able to live independently, or with support such as with their carers.

The main focus of the service is patients with Multiple Sclerosis, Parkinson’s disease, Huntington’s disease, Motor Neurone Disease and Epilepsy. However, this service will cover all patients with diagnosed neurological conditions where the reason for the referral is based on the neurological condition and the patient’s needs are best met in a community setting.

Patients are often seen in their own home but also clinics, in day care, workplace and residential care settings.

The service is provided by a multi-professional team including: Physiotherapists, Occupational Therapists, Speech and Language Therapist, Specialist Mental Health Nurse, Specialist Neurology Nurse, Dietician, Assistant Practitioners and Specialist Nurses for Multiple Sclerosis, Parkinson’s, Epilepsy, and Huntington’s Disease.

The core service hours are Monday to Friday from 8.00am- 4.00pm.

You can be referred by your GP or other health professional via our Health and Care Point on 0300 131 0300 or by using our online referral form.
