We support adults (18+) with complex health and social needs living in Nottingham. We offer an individualised service including one to one support, groups and phone befriending.  We work together with people living with mental health issues, enabling them to believe that life can change for the better.

This project is for people in secondary mental health services, at the point of discharge or those who have recently been discharged from services. 

For this project we accept referrals from workers including Community Psychiatric Nurses (CPN), psychiatrists, psychologists  and social workers.

The main focus of this project is to provide support, structure and enablement to people leaving secondary mental health care,  with the aim of preventing re-admittance to these services, by supporting people with:

Mental health – this could be support booking and attending appointments, advocacy, referrals and signposting to other specialist services

Debts – exploring the impact and source of debt, drawing up budget plans and support contacting specialised services for rights and advice with managing debt payments

Benefits – an overview of what support is available, help with applications, contacting and liaising with services, attending meetings and assessments, and enabling people to manage this independently

Unsuitable housing – support with applications for grants and referrals for alterations, furniture or equipment etc., support with moving house and liaising with housing services

Social isolation – assessing people’s needs, supporting them to build a social life and become part of the community including attending our monthly groups (art, walking, pool and craft)

We are able to offer one to one support for up to 12 months.

We are a partner organisation in this large project led by Framework. Funded by The Big Lottery and European Social Fund. Improving Lives counsellor offers support to this project.
