Other Essential Information

If you are a child or young person who is accessing support, or is about to access support, and you want to tell services/professionals about yourself, your views and what you want to happen, then you can download the children and young person’s passport here and write as much or as little as you want to in it. There is no right or wrong way to complete the passport.

This will belong to you and you will have control of who you share it with and how. The passport has been designed to help young people so that you don't have to keep re-telling your story or what you want professionals to know. Please bring it along to your Choice appointment with CAMHS if you have one, or to any meetings with professionals to support you to share with us what you want us to know about you. If you would like, this can then be passed to another professional/service so you don’t have to keep re-telling people the same things over and over again. We hope it will also help you to share things that you may find hard to say out loud.

We would really like your feedback about using the passport. To tell us what you like or don’t like about it or to ask us any questions please contact us (Martyn or Jack) on 0115 8764277.
